Heads Up Concussion Care provides structured concussion assessment and rehabilitation from Subiaco Physiotherapy, 8/531 Hay St, Subiaco WA 6008. We are Complete Concussions certified. Patients may book online without a referral. If you are referring from your practice, a referral letter is appreciated.

Patients may book online directly. For a clinical question about a specific case, contact the clinic. Private health insurance rebates are available on the spot for in-clinic appointments. For telehealth, patients can submit their invoice to their insurer.

When to refer

Patients who benefit from specialist input

  • Symptoms persisting beyond two to four weeks (post-concussion syndrome)
  • Return-to-work, return-to-learn, or return-to-sport decisions that need structured clearance
  • Vestibular dizziness, cervical headache, or balance symptoms after a head knock
  • Patient told to rest only, without improvement
  • Second opinion after normal imaging with ongoing functional impairment

We coordinate post-concussion syndrome rehabilitation, vestibular and cervical drivers, and return-to-sport clearance. Telehealth is available for suitable follow-ups.

Red flags: not for outpatient physio first

  • Loss of consciousness or deteriorating conscious state
  • Repeated vomiting or severe, worsening headache
  • Seizures, weakness, numbness, or unequal pupils
  • Suspected skull fracture or significant mechanism of injury
  • Increasing confusion, agitation, or unusual behaviour

In your consultation

Before you refer: what to cover in the GP visit

A structured GP assessment helps us continue care efficiently. At minimum, screen for red flags, complete a neurological examination, and use the appropriate concussion tool for the timing of the injury.

  • Screen for red flags and send to emergency care if present
  • Take a clear injury history: mechanism, loss of consciousness, amnesia, and symptom onset
  • Complete a neurological examination appropriate to your scope
  • Use SCAT6 for acute assessment (first 72 hours) or SCOAT6 when reviewing beyond day three
  • Document findings and attach completed SCAT6 or SCOAT6 results to your referral letter

Please include completed SCAT6 or SCOAT6 results with your referral where possible. That gives us a documented baseline and speeds up the first physiotherapy appointment.

Initial assessment

What the first appointment covers

Your patient receives a structured clinical assessment: symptom history, vestibular and ocular screening, cervical spine review, and cognitive load tolerance testing. All aligned with Complete Concussions protocols. We map an evidence-based recovery plan from the first visit, not passive rest alone.

Initial appointments are available in person at Subiaco Physiotherapy or via telehealth where appropriate. Concussion initial fee: $139 (GST-inclusive). Private health rebates apply for in-clinic visits.
